In consideration of Kiddie Crusoe, LLC ("Kiddie Crusoe") agreeing to reserve a space, date, and time for the registrant(s) listed here, the Parent/Guardian agrees to the following:
1. DURATION: Adventure sessions occur over a six (6) week span. Each six (6) week session commences with the first class that occurs after the date of registration. Dance adventures occur on an eight (8) week session. Trial classes are available by pre-registration. Up to two (2) weeks of make-up absences permitted per session to occur either the same week or in the corresponding week(s) directly following the eight week session. Arrangements must be made with the Adventure Leader prior to attendance. Please arrive promptly to your scheduled adventure sessions.
2. INCLUSIONS: As an Adventure registrant, each registered child can attend one (1) open play session per week Monday through Friday during normal business hours, special events excluded. Make-up open play sessions are not permitted after the prepaid class session has expired, unless occurring in tandem with a rescheduled session week. Reservations are recommended to ensure availability.
3. PAYMENT: Parent/Guardian agrees to pay for Adventure session and registration fee either in full at registration for a discounted rate or in three scheduled credit card payments as designated and selected in this form. Sibling discounts are already noted. There is a $20 Processing Fee for any cancellations, in addition to the cost of any classes that occurred up to the date of cancellation. There are NO REFUNDS after the start of week two (2) of the twelve week session. A $35 fee is assessed for declined or returned payment charges.
4. RELEASE & WAIVER OF LIABILITY and ASSUMPTION OF RISK: In consideration of being allowed to enter the play area and/or participate in any party, activity, and/or program at Kiddie Crusoe, LLC I, the undersigned, for myself and for the participants named below, acknowledge, appreciate, and agree to the following conditions:
I. We attest that I, the adult undersigned, am the parent or legal guardian of the participant(s) undersigned, or that we have obtained permission from the parent/legal guardian of the participant(s) undersigned to execute this agreement on their behalf;
II. We agree that we shall willingly comply with all stated and customary terms, posted safety signs, rules, and safety instructions and directions by Kiddie Crusoe and its staff and personnel as conditions for use of play equipment and participation in any party, event, activity, and/or program. In addition, if we observe any hazards during our stay or participation, we will immediately alert the nearest Kiddie Crusoe employee or official;
III. We acknowledge and understand that there are dangers and risks associated with the activities at/by Kiddie Crusoe, LLC and knowling and freely assume all such risks, both known and unknown, including those that may arise out of negligence from the persons released from liability below or other participants for me and my family's participcation; and
IV. We, for ourselves and on behalf of our family's heirs, assigns, administrators, personal representatives and next of kin, hereby agree to indemnify and release and hold harmless Kiddie Crusoe, LLC, their affiliates, officers, members, agents, employees, other participants, landlord, equipment manufacturers, and sponsoring agencies against any and all claims, injuries, liabilities, damages, or costs to person or property arising out of or related to our participation in any and all Kiddie Crusoe programs, activities, parties, the use of play area and/or play equipment to the fullest extent of the law.
V. This release and waiver extends to all acts of negligence by Kiddie Crusoe, LLC and is intended to be as broad and inclusive as permitted by the law, and if any portion of this agreement is held invalid, it is agreed that the balance shall, not withstanding, continue in full legal force and effect.
*I HAVE READ THE TERMS AS LISTED ABOVE AND THE RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT, FULLY UNDERSTAND ALL TERMS, WAIVERS AND ASSUMPTIONS, AND INCLUDE MY INITIALS VOLUNTARILY BELOW IN FULL AGREEMENT THEREOF.
